Vaccination Proof Required Prior to Your Procedure

Proof of vaccination is required to be brought to the Hospital during your pre-operative assessment visit or on the day of surgery.  Please note that procedures in Ambulatory Procedures Unit (APU), Endoscopy and Ophthalmology are currently excluded. 

You can retrieve proof of your vaccination record here.

All patients who are not fully vaccinated will be required to have a pre-operative COVID swab within 72 hours of their procedure. Please note that this includes  all patients under the age of 12.

All patients are also screened for COVID-19 symptoms at the time of their surgery.   A positive screening assessment will require pre-operative COVID-19 swabbing even if the patient is fully vaccinated. 


 

The Pre-Operative Planning Clinic is open to patients scheduled for a surgical procedure at our hospital. 

Location

Pre-operative Planning Clinic is located on Level 1 of the North tower in Ambulatory Care.

Hours of Operation

Monday to Friday 8 a.m. to 4 p.m.

Referrals

Patients are seen on referral from a surgeon. 

Physician

Patients are scheduled for an appointment on receipt of the Eventus booking slip.

Preparation

Most appointments will be virtual. In-person appointments are limited to pre-operative medicine and pre-operative anesthesia. 

In-person appointment

  • Arrive 10 minutes prior to your appointment.
  • Check-in at the registration desk. 

Virtual appointment

  • Follow the instructions provided in your virtual visit confirmation.
