Joseph Brant Hospital has partnered with Cancer Care Ontario and CAREpath, a subsidiary of Bayshore Healthcare Ltd. to offer CAREchart@home.

CAREchart@Home

CAREchart@home is an after hours telephone support service for patients undergoing cancer treatment. It provides symptom support for oncology patients receiving systemic and radiation therapy when their local treatment centre is closed.

Between the hospital and CAREchart@home, patients have access to 24-hour care, 365 days a year — allowing them to manage their symptoms while remaining at home.

  2. Have your Patient Information Sheet ready. The CAREchart@home nurse will require this information to assist you.

  3. An operator will answer your call and forward you to a CAREchart@home nurse for assistance. If the nurse is not immediately available, they will call you back within 15 minutes.
