Adult Mental Health Programs & Services

Joseph Brant Hospital's Community Mental Health services provide a continuum of care from assessment or consultation, to specialized treatment supports for clients with complex and serious mental health disorders.

General Psychiatrist Consultation Clinic 

The Psychiatrist Consultation Service provides assessment, medication review, and treatment recommendations via a "shared care" model with family physicians. 

 


 

Short-Term Treatment

Short-Term Treatment meets the needs of individuals who are experiencing mental health issues such as mood or anxiety disorders. 

Services include:

  • Assessment
  • Psychiatric evaluation
  • Treatment planning
  • Skill development
  • Coordination of community resources.

Clinicians also offer treatment via several groups, e.g. Cognitive Behavioural Therapy (CBT) for clients with depression or anxiety.

 


 

Intensive Case Management

Intensive Case Management Services offers comprehensive outreach services to adult clients and their families. 

This program addresses the multiple needs of clients with predominantly psychotic or severe mood disorders (e.g. bipolar) and focuses on the client's goals and effective community reintegration.  

 


 

Medication Clinic

The Medication Clinic facilitates the optimal use of psychotropic medications to ensure client stability in the community and supports clients in maintaining adequate functioning and an enhanced quality of life.

 


 

Phoenix Program-Early Intervention Service 

The Phoenix Early Intervention in Psychosis Program services individuals aged 14-35 years old who are experiencing early signs and symptoms of psychosis. This recovery-focused program helps individuals achieve their goals at work, at school and within their social networks, thereby reducing the impact psychosis can have on one’s life. The team includes psychiatrists, nurses, a social worker, an occupational therapist, a peer support worker and a substance use counsellor.

Services Include:

  • Psychiatric assessment and treatment
  • Medication and symptom management
  • Case management
  • Education about symptoms and treatments
  • Relapse prevention planning and strategies
  • Family education and support
  • Substance use counselling
  • Peer support
  • Assistance with education and work
  • Assistance with connecting to other community agencies

Accessing Care

People can self-refer or can be referred by a health professional or physician. To access the referral form or to get more information, contact Outpatient Mental Health Services at 905-632-3737 X3401.

 


Referrals

Referrals are accepted from physicians, hospitals, community agencies, and mental health and addiction professionals. There is an intake review to determine eligibility for all treatment services.

Centralized Adult Mental Health & Addiction Services Referral

 All referrals must have a completed PHQ-9 and GAD-7

Location: Joseph Brant Community Health Centre (JBCHC), next to the hospital 1182 North Shore Boulevard (east door entrance located just off North Shore Boulevard ) Burlington, ON Community Mental Health is located on the lower level of the Joseph Brant Community Health Centre.

Please enter at the east door entrance, take the elevator to the lower level and follow the Mental Health Services signs to the west side of the building, where our main reception is located.
