Complex-Continuing-Care

The purpose of the Complex Care program is to enhance health, functioning and quality of life, so that patients can safely return to the community. Care is inter-professional and colloborative, with patients and caregivers at the center. 

Complex Care Streams

The Complex Care program has three streams:

  1. Low-Intensity Rehabilitation
  2. Medically Complex
  3. Palliative Care

Low-Intensity Rehabilitation

Low-Intensity Rehabilitation is for patients who benefit from rehabilitation at a slower pace.  This program is goal oriented and time limited.

Medically Complex

Medically Complex is for patients who have multiple medically complex conditions who require unique support and care.  This is a short stay program.

Patients admitted to this stream maybe charged a co-payment to cover accommodation expenses in accordance with the Ministry of Health and Long-Term Care regulations.

Palliative Care

Palliative care aims to relieve or ease the symptoms, and the suffering of individuals and their families who face a life-limiting illness.

Admission

Application to the Complex Care Program is made by a member of the healthcare team to the HNHB LHIN.
