Joseph Brant Hospital takes the health and safety of our patients, visitors and staff very seriously. 

At Joseph Brant Hospital (JBH), safety is included in every aspect of care. 

Exit Signs

Emergency exits will be marked by green signage showing a silhouette in motion.

Identification Badges

Staff, volunteers and physicians are required to wear photo identification. If you are unable to see photo identification on the provider, please ask.  If you have concerns about the identity of a staff member, please contact Security.  

In Case of Emergency

Notify any staff member of your concerns immediately, including:

  • changes in health status,
  • fire or smoke,
  • noxious odours; and,
  • violent or threatening behaviours. 

Remain calm, return to your assigned room (this allows staff to account to your location and status), and await further directions.

Joseph Brant Hospital performs safety checks and bedside reports at shift change. These brief check-ins focus on your main concerns and immediate needs. Your daily care plan is also reviewed during bedside report.

Process

At the change of shift your nursing team will come to your bedside to:

  • introduce themselves,
  • ask permission to discuss your health; and,
  • plan your daily care.

Your nurses may need to wake you up if you are sleeping, unless you tell them otherwise.

Enhanced Safety

When nurses report to each other at your bedside, you are part of the exchange. The information shared keeps you informed about your care and provides the opportunity to ask questions.

During this time nurses also complete safety checks of your room, equipment and environment to make sure the area is safe for you.

Privacy

We value your input in your health care plan. If you are not comfortable with the report at your bedside or only want some information to be shared in the presence of others, please inform your nursing team. We respect your choice.

Unique Identifiers

Before receiving any treatment or medication, please confirm your name with the care provider and verify the correct treatment, procedure or medication is being dispensed.   The staff member will also verify a second identifier.  This may include; your hospital identification number, date of birth, telephone number, address, etc.

Medication Safety

  • Ensure that all of your doctors, nurses and pharmacy staff know about every medication you are taking. This includes prescription and over-the-counter medicines, and dietary supplements such as vitamins and herbs.   This information will be verified with you upon admission in a process called Best Possible Medication History or BPMH.
  • Make sure your doctors, nurses and pharmacy staff knows about any allergies and adverse reactions you have had to medications, supplements or food.
  • Ask questions if you are unfamiliar with a medication or if it looks different.  Make sure you know what the medication is being used to treat.

Discharge Safety

  • Ensure that you have been provided with instructions prior to your departure.  Know when your follow up appointments are, or who you should call to obtain this information.  Ensure that you have prescriptions and devices that you require for a safe transition to your home environment. 
  • Ensure that you have a friend/family member available to take you home from the hospital at the planned discharge time.
