Admitting & Registration is the first step to receiving care at our hospital. Admitting & Registration is responsible for collecting and confirming personal health information. This information is shared with your health care team to ensure safe, quality patient care.

Location

The central Admitting & Registration desk is located on Level 1 of the South tower across from the cafeteria.

Satellite locations are located within Ambulatory CareDiagnostic ImagingMaternal & Child and Surgical Services

Admitting & Registration supports in-patient and out-patient registration for:

Preparation

The type of care or procedure you are having will determine the steps you need to take to prepare yourself. You will receive this information from your physician or during your pre-operative planning appointment.

What to bring 

Documentation

  • Your Ontario health card.  If you do not have a health card, please bring government-issued photo identification. Such as a driver's license, passport, or permanent resident card.
  • Other insurance information if you do not have an Ontario Health card. Like, Out-of-Province Health insurance or Interim Federal Health Program. 
  • Information about private health insurance or other health benefits.
  • The names, addresses and telephone numbers of your family members, friends or other emergency contacts.
  • The name, address and telephone number of your family physician.
  • A copy of any advance care directive you may have.

Medications

  • All current medications in their original bottles. 
  • A complete list of medications you are currently taking. Including, prescription medications and non-prescription medications. As well as, vitamin or mineral supplements and herbal remedies.

Clothing and personal items

  • Loose, comfortable clothes that are easy to put on.
  • Hearing aids, dentures and eyeglasses.
  • Case for glasses or contact lenses. Lenses must be removed prior to surgery.
  • Socks and comfortable shoes with non-skid rubber soles, such as running shoes.
  • Walker, wheelchair or other mobility aids, labelled with your name and phone number. 
  • Anything that will make your stay more comfortable, such as books or other diversions.
  • If you are staying overnight, please bring toiletries such as a toothbrush and toothpaste, tissues, unscented soap, deodorant, lip and skin moisturizer. 

What to Expect

Once registered, you will receive an armband with your name and personal health information on it. The armband will be checked throughout your visit to ensure safe, quality patient care.

Fees

Patient may incur costs not covered by OHIP. Costs may include charges for accommodation, transportation, supplies or equipment. These costs are the responsibility of the patient and their family.
