The Rehabilitation program provides care to medically stable patients, including those who have experienced a cardiac event, stroke or surgery. The goal of the Rehabilitation program is help patients reach a level of independence and functioning that enables them to safely return to the community to continue their recovery.
The Rehabilitation program is transitional in nature. This program is goal oriented and time limited.
Location
Rehabilitation is located on Level 4 and 6 of the North Tower.
Referrals
Patients are received on referral. Referrals must be completed by a health care provider. To refer a patient for Rehabilitation, complete the Complex Care & Rehabilitation Application Form and fax it to 905-681-4849.

What to expect
Your Health Care Team
Rehabilitation is collaborative and interprofessional. While receiving rehabilitation, your health care team may consist of:
- Dietitian Services
- Nursing
- Occupational Therapy
- Physician
- Physiotherapy
- Pharmacy
- Social Work
- Speech and Language Therapy
Family Support
Families are important members of the care team. We encourage them to be an active participant in your rehabilitation.
This will assist with recovery in hospital and prepare for a smooth transition home.
Your Health Care Plan
Your health care team will work with you and your family to develop health care plan that meets your goals and needs. Through therapy and active participation, patients are able to track their progress toward these goals.
Fees
Patients admitted to this program may incur costs not covered by OHIP. These costs are the responsibility of the patient and their family.
