Joseph Brant Hospital offers a variety of different programs to help individuals with chronic obstructive pulmonary disease (COPD), including self-management outpatient support, educational seminars and hospital-to-home services.

Inspired
(Implementing a Novel Wellness House Program and Supportive Program of Individualized care for patients and families living with REspiratory Disease
- Holistic, proactive hospital to home care, providing patients/families living with moderate to severe stage COPD by a COPD Nurse Educator, Respiratory Therapist and Spiritual Care Counsellor (as needed)
- This program includes individualized self-management education; Action Plans; psychosocial support and advanced care planning
- 90 day program (includes telephone and home visiting support)
Integrated Comprehensive Care Program (ICC)
- Direct coordination of care for patient from hospital to home by an ICC Coordinator
- This program includes standardized care pathways; self-management education; home care and 24/7 ICC Coordinator support
- 60 day program (includes telephone and home visiting support)
Breathe Easy (Wellness House)
- Self management program in a small group setting facilitated by a COPD Coordinator and allied health team
- This program includes self-management education, exercise and lunch
- 20 session program running 2-3 half days per week from 11:00 a.m. to 2:00 p.m. for eight weeks
- Breath Easy is located in the Joseph Brant Wellness House, at 2160 Itabashi Way.
