Palliative care aims to relieve or ease the symptoms, and the suffering of individuals and their families who face a life-limiting illness.
The Palliative / End-of-Life Care program provides comfort and support to patients and families. Patient care focuses on maintaining dignity, quality of life, pain and symptom management and respect for each dying person while supporting families through this difficult period. This may include planning for a safe transfer to home or hospice through partnerships in the community.
Patients typically have chronic or end-stage diagnoses requiring frequent, skilled intervention and monitoring.
