The Heart Function clinic provides care to adults with a new diagnosis of heart failure and/or recurrent symptomatic heart failure.
Location
The Heart Function Clinic is located within Ambulatory Care, Level 1 of the North tower, 1N 600.
Hours of Operation
Monday to Fridays 9 a.m. to 4 p.m.
Referrals
Patients are seen by referral.
Physician or Nurse Practitioner
Referrals must be completed by a physician. To refer a patient for the Heart Function Clinic, complete the Ambulatory Care Referral Form.
Patients
Once referred, the scheduling office will call you to book an appointment.

General education about heart failure.

Self-monitoring strategies to reduce the risk of symptoms reoccurring.
- Medication adjustments
- Investigations
- Assessment by a nurse practitioner and a cardiologist.
- Assessment by a pharmacist, social worker or dietician, when requested.
Preparation
In-person appointment
- Ensure you have your Ontario Health Card or proof of medical coverage.
- Arrive 10 minutes prior to your appointment.
- Check-in at the registration desk.
- Bring a list of your current medications.
Virtual appointment
- Follow the instructions provided in your virtual visit confirmation.
- Have a list of your current medications readily available.
What to Expect
In-person appointment
- Your visit may take up to an hour or more.
- Blood and other laboratory work may be required depending on the assessment.
- Diagnostic Imaging may be required depending on the assessment.
Virtual appointment
- Taking part in a virtual visit
- Your visit may take up to an hour or more.
- Prescriptions will be faxed to your local pharmacy after your appointment.
