Birth Records Access

When your child is born, hospital staff will provide you with all the necessary documents to ensure your child's birth is registered.To order a birth certificate, you can contact the Office of the Registrar General in the following ways:

By Phone: 1-800-461-2156. Proof of birth is no longer available from the City of Burlington. In urgent situations you can obtain a birth certificate in an expedited manner through ServiceOntario. For more information you can contact Service Ontario at 1-800-461-2156 (Canada) or 416-325-8305 (GTA).

We consider all health care information that can be linked to an identifiable person as confidential. Disclosure of Health Information

Your family physician is considered an important member of the health care team. If you are receiving care at our hospital, a copy of your treatment record and/or diagnostic test result will be forwarded to your family physician unless you request otherwise. If you need to have information forwarded to another physician, you need to fill out the Request to Access Personal Health Information form and mail or fax it to us as shown on the form.

Recordings

To ensure the privacy of our patients, visitors and staff, consent is required for any photograph, recording or interview.  Without consent, the use of photography, video or sound recordings are not permitted on hospital property.

JBH believes in transparency and accountability.  If you are unable to obtain the information you are seeking through routine channels, you may consider making a request under the Freedom of Information and Protection of Privacy Act (FIPPA). Before you submit a written request for corporate records, check to see if the information you are looking for is already available on a public website.

JBH also publishes a Directory of Records, including the Index of Personal Information Bank as required by the Freedom of Information & Protection of Privacy Act (FIPPA). The directory is a list and detailed description of the general classes or types of records and personal information banks in the custody or control of the hospital.

To make a request for a corporate record, please complete the Access Request form and submit it with your application fee to the Freedom of Information Office, located in the Health Records Department at 905-632-3737 ext. 5683 or ext. 1277.

You should be aware that 30 days may be required to process your request and that fees are charged in accordance with the regulations of the legislation. In addition, you should be aware that there are a number of exemptions and exclusions from the right of access - refer to the Information and Privacy Commissioner's FIPPA Mini Guide for more detail.

We respect your right to access and/or correct your own health information. To access your information, you may speak to your physician or contact our Release of Information office in the Health Records Department at 905-632-3737 ext. 1465. You can also fill out the Request to Access Personal Health Information form and mail or fax it to us as shown on the form. Your request may take up to 30 days to be processed and that a fee (see below) is charged for release of records to patients and/or third parties (i.e. insurance companies and lawyers).

Fee Structure

  • Routine Service - Personal Requests (includes all personal and legal requests - $30
  • Insurance companies - $160
  • Demand Service (within 24 hours - additional charge): All of the above charges are to a maximum of 20 pages, each additional page is 25 cents. - $200
  • Supervising an individual's examination of an original record: - $27 per hour ($6.75 for each 15 mins per hour rate)
  • Making and providing a paper copy of a record from microfilm or microfiche: $ 0.50 per page, plus standard processing fees - $0.50
