Prioritizing Health through Acute Stabilization and Transition (PHAST) 

Prioritizing Health through Acute Stabilization and Transition (PHAST) is a made-in-Burlington service delivery innovation at the heart of a broader Full-Continuum Model for Mental Health and Addictions services.

The goal of PHAST is to provide a timely, centralized, user-friendly response to patients aged 16 and older during a mental health crisis, and provides options for ongoing care outside a hospital setting.

PHAST arose through a multi-agency partnership among leading providers of primary and secondary MHA services in Burlington:

Through PHAST, patients and families have rapid access to interdisciplinary assessment and stabilizing interventions to help them manage the crisis and transition seamlessly to next stage treatments and supports.

PHAST developed a novel therapeutic curriculum that has no specific beginning, no specific end, and no pre-requisites, allowing for rolling admission into the program and rolling transition to next stage services. This unique structure can accommodate significantly higher volumes of patients than typical urgent care programs.

 


 

Program of Assertive Community Treatment (PACT) 

PACT is a community outreach program, serving residents of the Region of Halton. PACT provides round-the-clock clinical treatment and rehabilitative and support services to individuals, in their homes, with a severe and persistent mental illness.

PACT is located in the Joseph Brant Community Health Centre, lower level of the Brant Centre, 1182 North Shore Boulevard, next to the hospital.

Referrals

Referrals are accepted from physicians, hospitals, community agencies, and mental health and addiction professionals. There is an intake review to determine eligibility for all treatment services.
