GIMRAC

 

The General Internal Medicine Rapid Assessment Clinic (GIMRAC) provides care for patients requiring a rapid assessment, in lieu of urgent care or admission to hospital.

Access

Patients require a physician-referral to access the GIMRAC. Referrals are typically obtained following a visit and/or discharge from the hospital.

Appointments

Patients will receive a call from the GIMRAC within 72 hours to schedule their appointment.

Patients will be seen by an interdisciplinary team,consisting of a:

  • Pharmacist
  • Internal Medicine Specialist
  • Registered Nurse
  • Outpatient Navigator

What to Expect

Your visit may take up to 3 hours depending on your needs. You may see multiple members of our team and several tests may need to be done.

  1. Initial GIMRAC appointment: The care team will evaluate your symptoms and create a care plan specific to your needs. After your initial appointment, the team may schedule additional tests and consultations with specialists.  
  2. Follow up appointment: if required, your internal medicine specialists will see you to review results. This may be the final visit or additional appointments may be scheduled.

Preparing for Your Visit

Please arrive 10 minutes prior to your appointment. Registration with the GIMRAC.

Please bring with you the following items:

  • Ontario Health Card or other coverage.
  • A recent “Meds Check” (available from your local Pharmacist) or ALL of your medications for the Pharmacist to review.

  • Any other personal care items you require or normally use during the day (e.g. snacks).

8:30a.m. - 4:30p.m.

The clinic is closed on weekends and statutory holidays.
