Community Care Access Centre (CCAC) 

The Hamilton, Niagara, Haldimand and Brant Community Care Access Centre (CCAC)  helps people of all ages return to their homes following surgery or a hospital stay. The CCAC provides information about community supports and services, arranges government-funded home care services, and facilitates placement into long-term care homes such as nursing homes.

The CCAC helps people get the services they need to reach their health goal, be it:

. Staying at home

. Avoiding a hospital stay or;

. Getting support when you leave the hospital

The CCAC knows the community well and can recommend services such as:

. Meal programs

. Support groups and;

. Other resources to help individuals live comfortably at home

Some services or programs in the community may have costs associated with them.

A physician's referral is not needed to access CCAC services.

The Community Care Access Centre is funded by the Ministry of Health and Long-Term Care through the Local Health Integration Network. Contact

The ConnectCARE Program is a personal response service that serves as a vital link between community and hospital once a patient goes home.

With ConnectCARE you wear a pendant or wristwatch style device that with the push of a button, connects you to caring, trained staff, 24 hours a day, 365 days a year.

ConnectCARE is hospital owned and monitored by Halton Healthcare Services, a proud partner of Joseph Brant Hospital.

Patient Transfer Ontario

Patient Transfer Ontario is a non-emergency patient transportation system.

The base fee for use via Ontario Patient Transfer is $165.55 for a 10km distance. For more information call toll free 1-866-527-9191.
