What is my responsibility when I am off due to a

Provide a Medical Certificate of Disability to Employee Health Services

MCD: CUPE, OPSEU, Non-Union, ONA - Hired after January 1st, 2006

Maintaining consistent communication with your manager throughout the disability or injury process is extremely important.

Failure to provide the appropriate or requested medical documentation when required will result in the withholding of sick benefits.

What are my obligations

Illness/Injury Certificate after the 3rd day of absence.
The Illness/Injury Certificate, based on a template developed by the Ontario Medical Association’s Committee on Work and Health provides information required to support us in assisting employees with their absence.
Similarly, a Medical Certificate of Disability (formerly known as an Attending Physician’s Statement) will be required after the fifth day of absence.  This form is required as per the Hospitals of Ontario Disability Income Plan (HOODIP) to support total disability and the payment of sick benefits.

What is a Medical Certificate of Disability (MCD)?

A Medical Certificate of Disability, completed by the employee’s Attending Physician is required whenever an absence due to illness:

  • Lasts five (5) shifts or more

  • Is prolonged and/or surgery is performed (in order to clear an employee to return to work)

  • Results in an employee requesting modified or alternative work

MCD: CUPE, OPSEU, Non-Union, ONA - Hired after January 1st, 2006

Will I be reinbursed for my medical forms? 

The cost of medical documentation requested to support absence will be reimbursed provided it is satisfactory to the employer and proof of payment has been received.
